Birth Certificates in Illinois
If you have misplaced your Illinois birth certificate, or if you never had one to begin with, you can request a certified copy. You can also request a copy of your child's birth certificate or a copy of a birth certificate for any children for whom you are an official representative. The state of Illinois does not consider birth certificates public records, so in general only authorized persons may obtain them. You'll need to provide proof of your identification along with your request. Records of births that occurred more than 75 years ago are available to genealogical researchers.

Download the Application for Search of Birth Record Files from the Illinois Department of Public Health website (see Resources).

Fill out the application completely and accurately. Write legibly. If you do not provide complete or accurate information, the Office of Vital Records may have difficulty finding your birth certificate.

Make out a check or money order to the Illinois Department of Public Health for the amount of $15 (the cost as of March 2010; check the Illinois Department of Public Health website in case the cost has changed).

Make a photocopy of your photo identification, such as a driver's license or passport. Your photo identification must be current, and the photocopy must be legible.

Mail the completed Application for Search of Birth Record Files, check or money order for $15 and photocopy of your photo identification to the Illinois Department of Public Health, Division of Vital Records, 605 West Jefferson Street, Springfield, IL 62702-5097.
